INCIDENT REPORT PUBLISHED: 21/10/2025 10:22. SERVICE: Some Teams Direct Routing services IMPACT: Loss of Service CONTINUOUS/INTERMITTENT: Continuous PRIORITY: P1 DOMAIN: Internal COMMONALITY: Incident affecting some Teams Direct Routing customers. REPORT: On 17/10/2025 at 12:10, it was reported that multiple Teams telephony nodes had become non-operational. Our engineering team investigated and identified that some configuration on our Teams SBC had been incorrectly modified during the deprovision of a customer on 16/10/2025. This prevented the connections from establishing when they refreshed. At 13:02, our engineering team rolled back the configuration to a known working state. All affected nodes were restored to full operation. CORRECTIVE ACTIONS: We have reviewed our customer off-boarding procedures and taken appropriate corrective actions. PERFORMANCE: In line with our Service Level Agreement, no Service Credits are available. UPDATE: 17/10/2025 16:15.
